Job Description

Reports to: Senior Manager, Family Health Team
Grade: N0:09
Hours: 37.5 hours per week
Salary: $41.82 - $52.28 per hour
Shifts: Monday - Fridays, 9:00am - 5:00pm
Status: Temporary Full Time (9 months)
Closing Date: February 3, 2025

Comprehensive primary care is the foundation of a high quality, sustainable and integrated health care system. The Toronto Western Family Health Team (TW FHT) is a multi-site, academic team composed of physicians, nurse practitioners, registered nurses, social workers, pharmacists and multiple other health care professionals focused on family and community health.

The Project Specialist, Business Intelligence bridges data, decisions, actions and outcomes to change how problems are solved, how data is visualized and how intelligent automation technologies support the strategic goals of the TW FHT. The role functions autonomously and collaboratively with the leadership and health professionals within the TW FHT as well as external partners. As an ideal candidate, you have proven expertise in Quality Improvement Decision Support, analyzing complex data and supporting healthcare organizational change and transformation initiatives, and thrive in a dynamic environment.

Strategic Leadership in Technology Solutions

  • Support the identification and identification of technology and/or innovation solutions that may improve patient care, staff productivity or data collection and ensure the appropriate key stakeholders are engaged throughout this process
  • Provide leadership for advanced technology solutions implementation that enhances patient care and operational efficiency.
  • Engage key stakeholders in a consultative manner to ensure comprehensive solution adoption
  • Manage and oversee the operation and integration of current IT platforms, including PSS, CHIME, OCEAN, EPIC and other IT utilized by the TW FHT

Data Management, Analytics & Informatics

  • Maintain and expand data analytics and informatics solutions to support the strategic and operational objectives of the TW FHT
  • Maintain best practice for data management and auditing processes.
  • Preparation, coordination and presentation of reporting requirements, integrating data into dashboard and reports
  • Provide supervision to EMR Administrator in regards to platform improvements, data collection and analysis, data clean up.

Quality and Project Management:

  • Manage and evaluate quality improvement initiatives in alignment with UHN standards, co chair Quality Improvement Committee
  • Support the development and refinement of performance indicators
  • Support the implementation of programs and services

Qualifications

  • Completion of a Bachelor's Degree program in a registered health profession or Computer Science, Computer Engineering, Health Informatics or recognized equivalent.
  • Master's Degree, preferred
  • At minimum, 4 years of practical and related experience, ideally in a primary care team based model
  • Minimum 1-2 years in working with and supporting enterprise level predictive analytics, dashboard development, simulation and reporting
  • Minimum 1 year of related experience in project management
  • Applied knowledge of TELUS Health (PSS) and OCEAN preferred
  • Experience in clinical quality and quality improvement preferred
  • Must be able to work independently and prioritize workload to meet deadlines.
  • Excellent relationship management and superb communication skills
