Job Summary

What you’ll be responsible for:

  • Promoting physical, emotional, cultural, social, and spiritual well-being.
  • Setting up supplies and equipment for CHW and assist with clean up.
  • Providing companionship and/or home-based leisure/ meaningful respite activities.
  • Assisting with and accompanying the client for mealtime activities.
  • Light housekeeping
  • Washing, drying, and folding laundry

What you bring to the role:

  • A Completion of Grade 12
  • A Class V Driver’s License and access to personal vehicle.
  • Food Safe is an asset, but online training will be provided.

What to expect during your first week?
You’ll be attending a 3 day orientation: one day at Seabird Island, one day at Stó:lō nation and an administration day. There will be policy driven learning and online learning within the first few days!

Where and when you’ll be working:
In the position, you’ll be providing care to the Stó:lō and Seabird Island nations. You’ll be travelling to and from clientele’s homes within the communities. Shifts are scheduled between the hours of 7am and 11pm based on community need.

Detailed Overview

As part of a multidisciplinary health care team, the Community Health Worker 1 supports client independence and promotes a safe work environment by providing assistance with various housekeeping duties and activities outlined in the care plan. Travel is a requirement of this position. Transportation arrangements must meet the operational requirements of FH in accordance with the service assignment and require the use of a personal vehicle and Valid BC Driver's License.


Responsibilities

  1. Observes clients and their environments, and reports behavioural, physical, and/or cognitive changes as well as unsafe conditions to supervisor.
  2. Participates in staff meetings, quality improvement initiatives, committees and in-services, and maintains up-to-date knowledge through involvement in appropriate work-related continuing education, as required by Fraser Health.
  3. Completes and maintains related records and documentation such as communication books and progress reports.

As per care plan:

  1. Performs housekeeping duties such as sweeping and mopping floors, vacuuming, dusting, washing dishes, laundry, bed linen changes and emptying garbage.
  2. Plans, prepares, and serves meals; provides assistance with clean up after meals.
  3. Assists with ordering and receiving groceries.
  4. Supports set up and clean up pre and post personal care performed by Community Health Worker 2 or caregiver, by performing duties such as ensuring towels, soap, etc. are readily available for bathing.
  5. Promotes a safe and healthy working environment by observing routine precautions and infection control procedures, removing obvious hazards related to safety, reporting faulty equipment, accidents, injuries and near misses, and adhering to rules regarding safety.
  6. Demonstrates methods and provides basic information to clients in relation to housekeeping, grocery shopping, meal planning and preparation, leisure activities etc.
  7. Provides short-term respite in the home, by performing duties such as sitting, socializing and engaging with clients in therapeutic activities (Strategies & Actions for Independent Living/SAIL exercises)
  8. Provides companionship by performing home based leisure activities with the client, such as engaging in conversation, reading with/or to the client, playing board games and/or card games.
  9. Performs other related duties as assigned within the parameters of the Home Support Guidelines.

Qualifications

Education and Experience

  • Grade 12.
  • Class V BC Driver's License and access to personal vehicle with appropriate business insurance coverage.
  • FoodSafe.
  • Recent, related experience of three months.
  • Or an equivalent combination of education, training, and experience.
  • Or other Qualifications determined to be reasonable and relevant to the level of work.

  • Ability to operate related equipment and technology (iPhones, computers).
